• Birth certificate - You must provide a copy of the child's birth certificate. We do not need an original.

 

• Physical/Medical Release - We must have a recent physical or medical release from your child's pediatrician/doctor or local health clinic.  Acceptable physical forms (if your doctor's office or clinic does not have the form) can be obtained from the KHSAA website.

• Photo - We must have a head shot of your child (a recent school year photo work).  Please write your child's name on the back of the photo.

• NKYFL Contract ** - Click the link for the contract that must be signed and turned in.
